North Dakota residents: Review the instructions and use this application to request a one time match of up to $300.


New Baby Match Application and Instructions

North Dakota residents: New Baby Match is open to every baby residing in North Dakota 12 months old or younger. Plan administrator and state-owned Bank of North Dakota will match up to $200 for the baby, earmarked for his or her higher education.


Designated Beneficiary Change Form

Use this form to change the designated beneficiary on an existing College SAVE account or to transfer a portion of the account assets to a new designated beneficiary's account.


Account Information Change Form

Use this form to change your address, phone number, participant, or successor participant information for a College SAVE account.


Limited Power of Attorney Form

Use this form to designate an individual, corporation, or other entity as your agent with limited authority to act on your College SAVE account.


Investment changes

Exchange / Future Contribution Form

Use this form to move money within a College SAVE account to a different investment option (allowed twice per calendar year) or to change investment options for future contributions (allowed anytime).


Payroll Direct Deposit Form

Use this form if:

  • You want to have money automatically deducted from your paycheck and deposited into a College SAVE account (your employer must offer this benefit).
  • You want to change your existing deduction.
If you're opening a new account, you can enroll online and select payroll deduction as your contribution method. You can also include your payroll deduction instructions on the Enrollment Form.


Automatic Investment / Electronic Bank Transfer

Start, change, or stop automatic bank transfer contributions; add or change bank information for electronic contributions to a College SAVE account.

Incoming Rollover Form

Request a direct rollover from another qualified 529 plan or Education Savings Account to a College SAVE account.

College SAVE is a 529 plan established by the State of North Dakota. Bank of North Dakota (Bank) acts as trustee of College SAVE Trust, a North Dakota Trust, and is responsible for administering College SAVE Trust and College SAVE. ABD, the Plan Manager, and its affiliates, have overall responsibility for the day-to-day operations of the Plan, including recordkeeping and marketing. The Vanguard Group, Inc.
