BND Match Application and Instructions

North Dakota residents: Review the instructions and use this application to request a one time match of up to $300.

Kindergarten Kickoff Match Application and Instructions

North Dakota residents: If your child is entering Kindergarten and you contribute $100 into a new or existing College SAVE account your $100 contribution will need to be received within 12 months of applying for the Match, or before your child turns 7 years old (whichever comes first) in order to receive the full match.

New Baby Match Application and Instructions

North Dakota residents: New Baby Match is open to every baby residing in North Dakota 12 months old or younger, and you can apply for the Match when you enroll and open a College SAVE account. Plan administrator and state-owned Bank of North Dakota will match up to $200 for the baby, earmarked for his or her higher education.

Designated Beneficiary Change Form

Use this form to change the designated beneficiary on an existing College SAVE account or to transfer a portion of the account assets to a new designated beneficiary's account.

 

Account Information Change Form

Use this form to change your address, phone number, participant, or successor participant information for a College SAVE account.

 

Limited Power of Attorney Form

Use this form to designate an individual, corporation, or other entity as your agent with limited authority to act on your College SAVE account.

 

Additional Purchase Form

Use this form to make additional contributions to your account by check.

 

W-9 Request for Taxpayer Identification Number and Certification

Use this form to certify the participant's or designated beneficiary's Social Security number or taxpayer identification number.

529 to Roth IRA Rollover Form

Request a direct rollover of 529 assets to the Beneficiary's Roth IRA at another Financial Institution.
